History:
Martin County Hospital District (MCHD) was organized in 1967 and has boundaries coterminous with Martin County, Texas. It is located in the heart of West Texas. Opened in 1949, the hospital was originally located near downtown Stanton. This facility served the community well for many years but was replaced with a state of the art 18 bed facility in 2012. The new facility is located on I20, and has been designated as a Critical Access Hospital
Summary of Responsibilities
The Chronic Care, Wellness, and School Based Clinic Coordinator is responsible and accountable for clinical expertise for specific patient populations. The nurse will oversee and perform disease management, assessment of disease states, care plan development, facilitation, and evaluation, education and referral to appropriate resources. Collaborates with internal and external resources or disciplines in an effort to achieve optimal clinical outcomes for the patient. Oversees and is responsible for the Chronic Care Management Programs, School Based Clinic Program, and other programs that promote wellness and prevention in specific and or vulnerable populations. Conducts outreach and coordinates events to promote wellness, patient empowerment, and prevention in the community and within the clinic. The coordinator will also apply business principles to reduce costs, better outcomes, and will actively participate in quality improvement processes as pertained to the Chronic Care, Wellness and School Based Clinic programs of the clinic.
Requirements:
Education
Graduate of a School of Professional Nursing - LVN or RN, BSN preferred
Licenses/Certificates
Must have a license to practice professional nursing by the Texas Board of Nursing or licensure from a state within the Nursing Compact.
Successful completion of AHA approved courses in BLS upon hire
Experience
One (1) year experience as a Professional Nurse - LVN or RN
