As a Wellness Nurse at Sunrise, you will play a vital role in ensuring the overall well-being of our residents.

Responsibilities

• Identify ongoing needs and services of residents to promote the highest quality of care.
• Participate in evaluations and documentation for residents experiencing changes in health care services.
• Assist the Resident Care Director in conducting evaluations and gathering data during resident move-ins and at regular intervals.
• Conduct monthly wellness visits for all residents.
• Monitor and follow up with residents who have experienced falls or incidents, documenting as required.
• Maintain communication with residents' families to ensure medical needs are met.
• Ensure weights and vital signs are obtained monthly for each resident.
• Provide clinical support to community team members as needed.
• Understand and follow infection control practices.
• Regularly monitor each resident's medication and treatment profile.
• Participate in quality assurance and clinical meetings.

Qualifications

• Graduate of an approved college/school of nursing.
• Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
• Minimum of one year of experience in home health, assisted living, hospital, or long-term care.
• Strong assessment skills and knowledge of nursing practices.
• Ability to handle multiple priorities effectively.
• Excellent written and verbal communication skills.
• Proficient in Microsoft Office and Sunrise applications.
• Willingness to work flexible hours, including weekends and evenings.
