POSITION SUMMARY: An outreach worker is a member of the SSTRY team who seeks to develop a trusting, mentor-like connection with youth and families in the community with substance use needs. The outreach worker will work directly with youth and families in their communities to enhance their motivation to change, sustain goals made in substance use treatment and/or to connect them to needed services and supports, including case management through the SSTRY model. The outreach worker is a resource to youth and families and can be called at any time to illicit support. They will also get to know specific communities and providers in the greater New Haven region so they can be viewed as a resource for individuals in need. The outreach worker will be accountable to the SSTRY Program Supervisor.

Accountable and responsible for the following results:

  • Will work within the SSTRY model to complete ACC (Assertive Continuing Care) requirements.
  • Will complete CRA/ACC training to be fully certified in ACC only.
  • Will work with youth at home and in the community.
  • Will complete SBIRT training
  • Will administer SBIRTs and complete necessary documentation
  • Will be able to work a flexible schedule.
  • Has a minimum of one face-to-face meeting or contact (phone, email, text, etc.) with each youth weekly.
  • Work with the family to understand the stages of change and set realistic goals.
  • Assists in engaging with youth and connecting them to case management services or other community-based support that promote recovery.
  • Maintain timely and accurate paperwork and record keeping for a needs assessment, service plan, client contact, case management, discharge plan and data collection.
  • Maintain receipts and records regarding expenses.
  • Attempt to engage youth who are resistant to participating in the program.
  • Participate in relevant program meetings, trainings programs, and events/meetings in the community. Effectively utilizes supervision.
  • Demonstrate sensitivity and awareness regarding cultural diversity as it relates to the client population.
  • Maintains care, welfare, safety and security through direct supervision of the clients, in addition to recognizing and correcting conditions which are, or could be, considered safety hazards.
  • Complies with agency confidentiality policies.
  • Models respect by being respectful of all staff, clients and self at all times and by following client rights policies.
  • Demonstrates sensitivity and awareness regarding cultural diversity as it relates to the client population.
  • Maintains care, welfare, safety and security of youth and self in addition to recognizing and correcting conditions which are, or could be, considered safety hazards.
  • Is aware of the Policy and Procedure Manuals and can access information as needed.
  • Maintains appropriate personal boundaries with young people and families served at The Children's Center of Hamden. Adheres to The Children’s Center of Hamden’s Boundary Code of Ethics.
  • Maintains all mandatory trainings including CPR, First Aid, CCG (Crisis Consulting Group), Boundary Training, Fire Safety Training, Driver Safety Training, OSHA (Bloodborne Pathogens, Infection Control, Workplace Violence, SDS), PREA (Prison Rape Elimination Act) and any others as assigned.
  • Performs any other related work assigned.

MINIMUM QUALIFICATIONS

Skills:

Knowledge of problems and needs of youth with substance use, recovery and behavioral health issues and ability to motivate young people to change. Strong ability to engage youth through a strength-based approach while communicating with respect to the client population. Ability to supervise groups of young people while enforcing rules. Ability to work as a team. Mature judgement in dealing with behavioral problems to avoid power struggles. Strong verbal and written communication skills.

Education:

At least 2 years of professional experience in the field of behavioral health for adolescents and/or transitional age youth and case management activities. BA/BS in Human services or related field preferred.

Previous Experience and/or Training:

Knowledge and experience working with adolescents. Bilingual in English/Spanish preferred.

LICENSURE /

CERTIFICATIONS

Required - Valid Connecticut Driver’s License with Safe Driving Record

BA/BS in Human services or related field preferred.
