Job Summary:
The RN Preceptor for Home Health serves as a mentor and clinical instructor for newly hired clinicians transitioning into home health care. The preceptor supports orientation and onboarding by providing hands-on training in the field and within the EMR system. This role ensures safe, effective patient care while orienting home health staff.
Key Responsibilities:
Orient and train clinicians to the home health environment, including clinical workflows, documentation, patient interaction, and safety protocols.
Guide preceptees on the use of KanTime EMR, ensuring accurate and compliant documentation of visits, OASIS assessments, and care coordination.
Accompany new staff on home visits to model skilled home health care, conduct assessments, and provide real-time feedback.
Educate on agency policies, procedures, infection control, and emergency protocols specific to home settings.
Promote best practices for time management, visit planning, and patient education.
Evaluate performance and progress of preceptees and communicate findings to clinical leadership.
Support the transition of new clinicians into independent, confident practitioners.
Stay current with clinical standards, Medicare guidelines, and KanTime updates to ensure accurate instruction.
Assist in refining and updating preceptorship tools and orientation materials.
Requirements:
Qualifications:
Active and unrestricted Registered Nurse license in the state of practice.
Minimum 4-5 years of clinical nursing experience, with at least 2 years in home health.
Proficient with EMR, experience with KanTime preferred.
Experience with OASIS documentation and Medicare home health regulations.
Prior mentoring, precepting, or leadership experience preferred.
Current BLS certification.
Valid driver's license and reliable transportation.
